Extended Care Rehabilitation Center
ECRC services include:

Short term rehabilitation (wound care, occupational therapy, kinesiotherapy, physical therapy, speech therapy, skilled nursing)
Long term skilled nursing, dementia, psychiatric, and maintenance care

Respite care

Home and Community Based Care
This consists of a range of programs that promote and manage care in veteran's homes and other community settings. Services include:

Home Based Primary Care
HBPC provides intensive case management to patients with chronic high risk medical conditions who have frequent admissions to acute care or visits to the Emergency Department. An interdisciplinary team comprised of Registered Nurses, Nurse Practitioners, Social Worker, Nutritionist, Clinical Pharmacist, Occupational and Physical Therapists, Psychologist, and physician strive to reduce the need for unplanned medical interventions.
Care Coordination/ Home Telehealth
CC/HT is a means for primary care providers to more closely monitor the progress of patients with chronic disease processes and to assure ongoing patient education about their conditions. To accomplish this, interactive electronic communication devices are placed in patient's homes and responses or telemetric data are recorded and tracked with outliers reported to the physician.
Other
Geriatrics and Extended Care at the VAMC in Salisbury is pleased to serve as an educational site for students of medicine, pharmacy, psychology, social work, nutrition, recreation therapy and nursing.
